Abstract: This article discusses an incident that occurred where a woman was killed and a young male was severely injured in a fire. The cause of the fire was finally determined to be a replacement plug on a light. The process by which this determination was made is detailed in this article and is presented in an effort to raise the level of awareness regarding this particular type of replacement plug.
